Someone pointed out Irreversible earlier and I think it takes the cake. I haven't seen Salo but I reckon it's a pretty tough watch too. The first movie to truly fuck me up was Cannibal Holocaust.
I also recently saw Bone Tomahawk and found it pretty heavy. Just by the way the violence was handled, it was graphic but yet realistic at the same time. Just not having any kind of music during a violent scene already makes it more effective.

A car accident where a dude and a woman crashed into another car, there was blood everywhere and they had their torso nearly crushed by the dented metal parts. It wasn't even the visual part that got me, it was the sounds, i.e. the screams of genuine agony and screams of people crying for their life. I don't know if they survived, but from what I remember it looked pretty fatal.
