There's multiple layers to it. First of all, Tony partially did not believe he had dementia. He thought it was still mostly just an act because when he acted that way in the first place, it saved him from prison etc and ever since he started using the "dementia" excuse people started siding with him and Tony thought he was playing it up as an excuse. At that point in the series Tony thought everyone was against him and when Junior shot him, he initially saw it as a power play to get control of the family.

Second of all, Tony mentions this in another episode in an earlier season - "If you really can't control what you do or say, why can't you say something nice instead, why does it have to be hurtful". In essence the whole series is Tony dealing with never having any real role models or love from the men he looked up to and constantly seeking their approval. So when Junior kept saying "You never had the makings of a varsity athlete" he kept getting pissed off at Junior, even when he knew he was sick, because he thought that the reason Junior was saying these things was because he subconsciously didn't respect or love Tony, that's why Tony straight up asked him in one episode "Don't you love me?" and Junior started crying because he loved Tony so much, but did not know how to express it and realized he was sick and couldn't control what he was saying, even to someone he loved as much as Tony.

So when Junior shot him, Tony saw it as "He was sick, but if he loved me the thought of doing it would never have crossed his mind, he must subconsciously hate me".

And the third layer was simply that Tony was a brainet and "gangster" and they don't really understand or believe in mental illness like that.
