Case History:

Two months prior to presentation, the patient had applied dry ice to his feet as a method to relieve emotional distress. On the day of presentation, the patient had unintentionally fallen asleep for six hours while his feet were wrapped in dry ice. This prolonged thermal trauma resulted in rhabdomyolysis and tissue necrosis that required critical care. Once the patient was stabilized, plastic surgery discussed management options with the patient. Bilateral below-knee amputation and conservative limb-sparing care were both considered medically appropriate. During the discussion, the patient expressed immediate enthusiasm for amputation. The patient’s unusually eager response raised concerns that the accidental frostbite had actually been an attempt at self-amputation. Psychiatry was consulted to evaluate the patient’s capacity to consent for the amputation. The patient screened negative for suicidal ideation, psychotic symptoms, and mania. During mental status examination, however, he demonstrated an inappropriately euphoric affect while discussing his future as an amputee. The patient reported that he had known many veteran amputees during his previous military deployment in Afghanistan.
