To actually answer your question, yes underwear is part of the wardrobe department's responsibility. It doesn't necessarily need to be featured in the scene. This one commercial I worked on this actors dick was flopping around really obviously in his gym shorts so the assistant director literally said over the walkie talkies "hey wardrobe can we get a jock strap next time" to keep his bits and pieces in order.
