I think the "angriest boy" book and doll are meant to be real, the implication is that they were coopted by Farouk as a tool to keep messing with David.
Though I'm unsure how much of what David's mother experienced was real and how much was some kind of mental palace made by Xavier, much like the mental space where David and Syd could touch each other, have sex, etc.
