Usually through a kind of flat overreaction, Catelyn telling Robb in front of all his bannermen that she wants him to know what it's like to kiss a girl (poor attempt at trying to portay Catelyn's motherly love and it doesn't make sense for her to speak this way when the book is about trying to hone in on the subtle and important aspects of power and how it's maintain, Catelyn's meant to be experience at court and these lines make her son look weak.) The scene of Catelyn obsessively making her doll shows us much better that she's a loving mother for example.

None of that needs to be said, we should expect to know that Jaime would have been shocked by what we've learnt about his character and the way he behaves in response to getting called something that shocks him, and it's also overdoing it to try and make out he's so particularly shocked by a bit of mockery, it's a poor attempt this time to try and remind us he's proud and revered. GRRM writes internal monologue as if it were descriptive narration of the setting, rather than really getting the personality of the character to come through.
Examples aren't really the best way to make a point in reviewing of course because I can't take every line of dialogue and sort it into ones that work and ones that don't and post it here, and there'll be "yeah but, that other time!" responses, but I can say that on balance there's a lot more of this weak stuff coming through. The first book is often criticised for a lack of humour, they don't mean a lack of jokes as such but what they're really getting at though is a lack of personality, and everyone talking and thinking in sincere descriptive ways. Cersei is completely written as an angry queen for a very long time with no complexities.
