GRRM told D&D how Game of Thrones ends. He told them the major plot points and he told them who lives and who dies. That is exactly what happens in the show because the show followed his notes. Characters might change and locales might be different, but what we saw is what will happen.

The fact that the mother is never described, the ice and fire mention when neither Targs nor Martells have anything to do with ice and that Rhaegar looks directly at the camera when mentioning the "dragon has three heads" thing makes me think that it's a vision of baby Jon with Rhaegar and Lyanna and then Rhaegar tells Dany to find a third "head" besides her and Jon.
