tl;dr is this guy wakes up in his space ship and finds an old love interest. There's some sex and stuff, she tells him he's stranded, blah blah. A bunch more happens, more sex, then she reveals to him that he's actually in a simulation that she is keeping him in so he doesn't have to realize just how terrible the situation is. He demands to be let out. She is sad because she really does care for him and want to keep him safe and happy, but he demands again. She lets him out, he wakes up and sees this figure coming out of the shadow. This is the thing keeping him in a simulation

It's based on a short story and funny enough, Greta (the spider monster) is actually unironically benevolent and really does care for him and all the people that get lost in space.

What I didn't get was:

If she honestly cares, then why is everyone is such terrible condition? Why does his crewmates die? Why specifically the chick that wakes up and knows that they are in a simulation and tries to attack Greta?

The crewmates died because they modified their little grav sleeping couches by painting it. It's said in the short story that the paint flakes caused problems. It's not a terrible condition either, for her. I mean, she's a spider. She cant make a Sheraton hotel or something, so she can only make a spider nest. That chick who woke up was actually Greta's simulation, not the chick waking up. This crew is the first time Greta ever met humans, and she wanted to see how Thom would break the news to the crewwoman. That way Greta can try to learn from that and break it to Thom easier.

It's not said. After the reveal and Thom going mad, Greta puts him back into the simulation from square one and tries again. That's the end of the story

By terrible condition I meant Tom. He is completely emaciated, his teeth are falling out, his hair is oily. He's clearly dying. Yet she "cares"? Come on.

She's not a doctor, she also can't whip up food. She can only provide what she has, which is spider webs and whatever there is on other ships that get stranded. In the short story it's implied that conditions are a little better and there are more than just her on the nest station, and that once people wake up they kinda get into a community and learn to live there. That was probably too much for a 15 minute episode

That's the indication they sort of hint at in the show, but in the short story it's very explicit that she is actually benevolent. There's no ambiguity in her intentions. If she was lying to him, why bother going through the wake up procedure? As soon as he started asking questions, why not just reset the simulation? Why have the fake crewgirl wake up and question things, to cause Thom to? There's no reason to do any of that if she is pulling off a Matrix kinda thing"
