He's not that great. Game of Thrones' score is nowhere near as good as Lost or BSG.
People rave about him, but in my estimation he's just ok. He's better than what you normally hear in American TV shows, which is so bland that nobody even pays attention to it, but that doesn't make him great.

From my knowledge the composer makes the music after they watch a scene. It would have been nice to see even some different type of leitmotif at the very least.

He was a good match for an HBO miniseries. When GoT got big and everyone expected more, he just wasn't up to the job. and are right. The motifs were mostly forgettable and limited the dramatic potential of the show. The last episode was particularly bad where he just remixed the main theme in several parts. It works because the main theme, and many of his other pieces, are simple ostinato works that are amenable to reworking. Judging from GoT, though, he is a B tier composer at best. In fifty years, film music concerts will still be playing John Williams and Morricone but I doubt people will even remember Djiwadi.
I don't blame the producers for not getting the best. I don't even know if there are any top tier composers in their prime left.

By music direction I mean when to use music and what music to use.
I guess I didnt explain enough.
Most composers have music direction. They will watch the movie or show, and sometimes even make music on the fly. Composition is often the very last thing to be done in the production process. For example Hanz Zimmer was the composer for Dark Knight and music director. He literally watched the film and used his keyboard at the same time feeling it out.
That being said directors will often always have final say.
If they're re-using the same track seven times then surely that wasn't the composer's decision.
Starks, Lannisters, Greyjoys', Robert's themes are memorable.
He’s good at meme composing, as in his songs easily become memetic even within the work itself. So he plays off of three or so pieces of his work that became iconic to good effect. Honestly the perfect composer for in universe meme songs like Rains of Castamere

I can only somewhat recall the Lannister's theme whereas I can recall nearly all the themes in Star Wars, Star Trek, Lord of the Rings, the main themes to Robocop, Jurassic Park, Forrest Gump, The Rock, The Good, the Bad, and the Ugly, The Mission, Love Story, Independence Day, The Hunt for Red October, The Rocketeer, King of Kings, Gone with the Wind, etc., even though I haven't heard those pieces in many years whereas it's been a month since GoT ended?

I always imagined the pulsing to be teh blood in the Governors head. The rush he gets from hurting people, the rush he get before beheading Hershel. Its also the first time they use synthsized sound in the show, which I would say matches with the chaninging of scope. No more are the zombies a threat. Its the governor.
I have no reason to doubt that it works and that there are intellectual justifications for it, but as a piece of music it's underwhelming.
