But I've been thinking about it for a couple months, and something like a zombie apocalypse or other similar disease-based disaster could set it in motion.
The cities are more dense, liberal, and dependent on sophisticated infrastructure to function. What's more, they're substantially more susceptible to a biological disaster. A major wave of plagues would destroy the left's voting power, and any people that survived would become refugees as cities would no longer be functional. Food would become a coveted resource, so rural communities (who already have a culture of being well armed), would have to constantly patrol the countryside warding off starving city slickers. Eventually, between the looting and threat of spreading disease, any surviving rural communities would be forced to shoot outsiders on sight. Martial law would be declared well before this, of course, but it effectiveness would be questionable.
By the time the disease is neutralized, only ultra-conservative communities would survive to inherit whatever's left of the military dictatorship of a government.

IIRC the author of the book wanted to do a hypothetical scenario where the muslim takeover of Iran happened in the US. So since the biggest religion in the US is christianity she used that. And since the scenario is very far fetched she added in the fertility crisis. I'm 99% certain the showrunners don't realise this though.

The point is about oppression of women, in both the book and the show. It's "oppression of women is bad" not "islam/christianity is bad". Atwood taking inspiration from the world, Iran in its treatment of women being one, but not exclusive (Afghanistan, Puritan New England, Cambodia, and Romania were all cited by her as inspirations) thus should not be conflated with the Muslim religion, or Christianity for that matter, as a whole, as you and many others seem to be doing.
