Soviet reactors were extremely stupid designs, no containment vessel (although it's not what makes them more likely to fail, it does mean they make a bigger mess)
Old American reactors are stupid too but much less stupid
Nuclear reactors are fine if you design them and manager them right
The challenge is cores continue generating a quarter of their heat for days after the reaction is shut off and need coolant fed to them constantly to keep the core from getting so hot it melts itself
if the active coolant feed stops because of pipe or pump failure the coolant boils and the steam explodes the reactor apart (Fukushima, Three Mile Island) and possibly the remaining core melts down if left unattended

Most of these problems come from using water as coolant for a solid core, expensive molten core reactors where the fuel is dissolved in a liquid are safer. Can't melt down because they're already molten and you can shut down the reaction by increasing the surface area or decreasing the pressure or both (like passively draining the liquid core into a large reservoir when the coolant pumps fail)
