The world got fucked over by a destructive dimensional rift called the Death Stranding.
The US got completely crippled and became the United Cities of America. The former president, Amelie, wants to reunite the cities for the future of humanity. Sam, a transporter under her employ, doesn't believe that a unified government can ever work and likely wants to maintain the status quo. Meanwhile a gang of terrorists who call themselves Homo Demens (roughly meaning Men who've Lost Their Minds) are taking advantage of the dimensional rifts and the creatures that spawned from them to fuck shit up, likely because they feel that madness is the next step in human evolution.
The primary mission of the game is to get Sam to take whatever Amelie asks him to in order to reuinite the US and potentially save the world in the process. Sam himself doesn't think this goal is viable, so there may be multiple endings.
As for the Bridge Babies, that's another way they're taking advantage of the rift. The Bridge Babies are conduits to the "other side", which I think is past the fourth wall in the real world, a metanarrative thing. The baby represents the actual player of the game, which would explain why the player can see the ghosts even though Sam can't.

I think, and this is pure speculation, but I think the Homo Demens don't use strictly bridge babies as a way to get in touch with the "other side". Higgs was talking about Cliff as being part of the other side himself, and it seemed to imply that they were dragging the war shit in with them, or at least found a way to use what was already there. This could even be what the fake bridge babies are for.

Weird as the bridge babies are, it's actually a pretty smart way to go about it from what I'm seeing. They have no real way of communicating with the player, but they do know what the player does and prioritizes their defense. This doesn't mean you're not controlling Sam, though, obviously, but it does put you in the game without really putting you into it. They know about as much about the player as they reasonably should.
It's only barely scratching the fourth wall, too. It's clever.

Gameplay-wise it's looking like what I was hoping for: MGSV but with stronger survival elements, light mountaineering, bosses, and a finished story. That it's a new IP is even better. I'm probably more hype than I should be.
What's got me actually curious is how the plot unfolds and how the fucked dimensional shit operates. The fake bridge babies (bridge dolls, I guess) are extremely interesting to me. Another odd thing is how different Mads is when he's talking to what I presume is the player's BB and when he's on the attack as a soldier. I'd even say he's playing two different characters if he didn't have blood on his face in the beginning of the trailer.
This, but unironically. If a game's gonna involve climbing mountains, I'd rather use shit to climb them rather than walk up the mountain sideways.

it feels like the game will have a few separate tones of gameplay, where part of it's about world traversal to deliver packages (this will likely be side missions to get you better gear) interspersed with stealth sections from either humans or the floating dead guys, and if you get pulled in by the dead guys then it becomes a shooter until you can escape
the more the trailer went on, the harder it became to narrow down a genre, hell he even showcases a dodge move and melee combat too
