Monster. Pretty good story about a surgeon who going against orders saves a kid from dying (he was ordered to operate on a more important patient, but he stuck by the kid). Many years later the kid turns out to be a sociopathic murderer spreading death through Europe. The doctor feels responsible for it and abandons his job to go hunt the killer.

OP's picture is the killer, guy named Johan. The impossible casting part comes from the fact that he has a twin sister and they look so much alike he successfully impersonates her for a while to advance his plans.

One of the issues of casting characters like Johan and Griffith is how young they are. Both spend most, if not all of their respective series being 18-22.

Hollywood often casts 20~30-year-olds to play teens. Moreover there is no reason to be absolutely faithful to the source material. Anime can be pretty stupid about ages.
