Heirarchy is actually a recognised and real human factors problem that can override common sense. We just have a strong inbuilt tendency to submit to authority decisions that fly in the face of what we know.
United 173 for example ran out of fuel and crashed because the pilot lost track of how long they'd been in a holding pattern, the other crew could tell they were running out of fuel and kept trying to hint at it to the pilot but because of the heirarchy problem never felt confident enough to push the issue when the pilots didn't seem to think it was much.
