* The xenon poisoning was mostly related to running high power for grid demands then dropping fast for the test. They were supposed to drop power levels slowly. When the grid manager demanded they maintain output they should have postponed the test.

* The steam voids formed during the test due to the change in water flow.

* Once the steam voids formed spiking power the neutron flux increased very quickly "burning off" the xenon, creating even more reactivity. So in a matter of seconds you've gone from low power to 'one more straw on the camel's back' and that straw was likely the control rod insert. (I say likely because there's some debate on whether or not the first explosion occurred before AZ-5. But it seems likely that AZ-5 actually triggered it, as portrayed in the series.)
