We also have the one about how a girl trips on a rope and skins her knee and the scrupulous doctors "treat" her by slicing off most of her body and harvesting her organs and replacing it with low quality machinery, which of course she can't afford at all so to pay for her treatment debt she goes out with a rope to trip unsuspecting people to repeat the cycle.
Then we see this happen first hand later on in the story. So I think the comical extras are still mostly canon world details even if played for laughs.
