Simple terms,

Most reactors use water of some sort to cool the reactor down. (Some use air or carbon dioxide) and i that water turns to steam, it slows down the reaction. This is a built in safety feature of the likes of a western BWR.

However in the RBMK design water turning to steam causes the reaction to speed up.

They were doing tests to test what would happen when the pumps failed, and instead of doing it at the correct power of 700 MW, they were doing it at 200 MW with none of the control rod in the reactor. This didn't give enough power to to the pumps to keep rotating till the backup generators started to pump. Thus water turned to steam and there was a huge power spike.

They tried to shut down the reactor using the emergency stop, but due to a design issue it gives a short term boost to power, causing it to exceed the power by 10x the design limit, causing a huge steam explosion.

This was quickly followed by a 2nd explosion, which was one of two things, a larger steam explosion or a full scale prompt critical that caused a huge explosion. this opened the core to the outside and also caused a fire, and since the gas seals were broken, oxygen could get to the hot graphite, which started to burn and released buttloads of radiation.

every single safety measure was disabled, they reduced coolant to the reactor as a test, and then the reactor started to overheat and run away. When they hit the emergency shutdown button, the rods inserted had a side effect of heating up the reactor before cooling it down. Imagine if you were to extinguish a fire in a way that makes the fire extremely hot before it goes out. Anyways, the reactor got so hot that the steam became superheated and the explosion was actually a steam explosion.

they ran the reactor for a long time at low power while they were preparing for their test, this caused a chemical called xenon to build up on the fuel rods making the power go really low; too low for the test. To get the power back up fast they did stupid shit like removing control rods and disabling safety measures. The xenon burned off and the power spiked. They tried to start the emergency shut down but this didn't work for several reasons
1) they manually removed most control rods so the automated systems couldn't put most of them back anyways
2) SCRAM is the emergency shut down, the poor design of the reactor actually cause further heat increase during emergency shut down
3) the system seized up before it went all the way back in anyways

the massive and sudden rize in head vaporized all the water in the tank in a matter of seconds and blew everything apart. it did not blow up like a nuclear bomb, it couldn't, it was a steam explosion that spewed radioactive debris everywhere.

The control rods in the RBMK reactor are basically giant matches, and when the tip goes into the overheating reactor the head of the match ignites and blows the whole reactor.

In the Swiss report it wasn't a fuel rod and he likely would not have survived being that close to a fuel rod. As I recall it was tubing that got caught when the rods were being moved.

As I recall those divers are safe as kittens at 3+ meters from the fuel rods. But in the time it would take to swim 3 meters down and immediately back up you would be dead.

Multiple design flaws colliding with poor operation and management. Fuckin' infodump time me hearties. This is the best explanation I can give as a civil engineer who has trawled wikipedia and many other sites.

The story begins with the Core Containment Vessel. This is the last and best defence a reactor has against meltdown - a solid steel bottle with a belly full of concrete, housing the entire core. Even if the core melts down it is designed to be unable to penetrate this structure. It works - even in Fukushima, none of the CCVs were breached.

The Soviet union didn't have the technology to construct Core Containment Vessels. Making a contiguous structure that thicc and that big is hard. So they designed a reactor that didn't need one - the Pressure Tube design.

Using hundreds of individual cooling channels instead of a single massive one in which the reactor core is immersed, Pressure Tube reactors disperse the core material over a wider area so that even in the event of a criticality accident, actual core meltdown isn't possible - there just isn't enough heat concentration. It's a brilliant workaround!

Except that's bullshit.

The Pressure Tube system is just as vulnerable to meltdown as any other reactor - in fact, far more so. The maze of individual pipes inevitably causes hydrodynamic flux, creating pressure instability, which greatly increases the chance of steam pockets. Steam pockets are VERY VERY BAD - water in the reactor serves a dual purpose of taking heat off the reactor so it doesn't melt, and absorbing free neutrons so it doesn't get too hot. Steam is lousy at both of these tasks, so if the water in your reactor is replaced by steam, your reactor is going to get very hot, very fast.

Now, for the test itself. The test was simple - determine if the reactor can run its' own water pumps in low power conditions in the event of a loss of external power. This was intended to be done via momentum in the large steam turbines. The operational parameters were that the test would be 45 seconds long - the length of time it would take for the backup diesel generators to start up - and be performed at the relatively low power level of 700 MW.

But things started to go wrong. Originally, the Day Shift was to conduct the test, but it was decided that it would be unnaceptable to disrupt Kiev's power supply, so the test was moved at the last minute to the less-trained Night Shift. They dithered in preparations and let the reactor sit at low power for a long period of time. At about 1 AM, power started falling continuously, and bottomed out at about 40 MW.

The guy directing the test (our man Dyatlov) was furious. He was eyeing a promotion so he would get a cushy desk job where he wouldn't have to deal with a room full of workers all day, so he was determined to get the test right. He ordered nearly all of the 211 control rods pulled out of the reactor to restore power to the correct level.

With all but 12 or so rods removed, power climbed...to 230 MW. Dyatlov said that was fine and ordered the test to start, threatening the control room staff if they disobeyed him.

You can probably guess the reactor was badly Xenon poisoned by now. When the test started, external power to the water pumps was switched off, reducing circulation of the coolant - this increased the pressure instability inherent in the reactor.

At this point I have to start speculating, because all the evidence of what actually happened is either destroyed or entombed in the Sarcophagus.

At approximately 1:23 and a bit, a steam pocket formed in the reactor core. Steam being a shit coolant and absorber, this pocket heated up enormously, burning off the Xenon and turning the whole reactor hot as hellfire - nearly doubling power output instantly. The engineers panicked, and pressed the AZ-5 button.

AZ-5 initiates the SCRAM procedure, plunging all the control rods into the reactor to shut it down RIGHT THE FUCK NOW. But remember what I said earlier...about inserting the rods too quickly? Yep. The SCRAM caused a heat spike instead of heat loss, and the temperature in the reactor became so high the graphite fuel rod channels fractured, jamming the control rods in place.

Now the reactor was doomed. The heat continued to increase, boiling off all the water in circulation and increasing the pressure to the point where it blasted the 1000-ton reactor lid out of its housing and clean through the roof. This was the first explosion - a release of steam. The lid came back down and was left wedged sideways in the core chamber.

A few seconds later, there was a second explosion - exactly what this was is unknown, but it resulted in the ejection of a large amount of core material (graphite, fuel rods, et cetera) into the air, where it rained down on the roof, the grounds around the building, and back into the reactor hall. The prevailing theory is that this was a result of the steam interacting with the zirconium fuel rod cladding, creating hydrogen, which then combusted when it reached the correct fuel-air mixture.

Whatever happened, the reactor exploded.

Thyroid naturally absorbs iodine. Thyroid cannot differentiate between normal and radioactive iodine. Saturating it with iodine will keep it from absorbing radioactive iodine in the environment.

How helpful it is depends on the source/type of the radiation and how close you are to it. If you are far away it will protect your thyroid which is very susceptible to radiation damage and hopefully whatever other radiation is present is low enough that it won't kill you. Xenon-135 is a fission product of uranium and absorbs neutrons. Absorbed neutrons can't hit another fissile atom...another atom of uranium...to continue the chain reaction.

But it also 'burns off' quickly during normal operation. It's not literally burning, it becomes stable once it absorbs a neutron.

If a reactor is loaded with unstable Xenon-135 it presents a problem to continued reactor operations. It doesn't make an excursion more likely...quite the opposite. But if you pull all the control rods to counteract it, THAT makes an excursion likely as soon as the Xenon-135 starts 'burning off.'
