Chernobyl the main issue was beta particles.
All these types of radiation can cause cell mutations by damaging DNA, in addition to their immediate cell damage/burns.

I was disappointed that it was in English at first as well but it makes sense the more I watch the show. It's less about straight-up historical realism and more about these subtle power plays and plans against the social order of the communist government. They're English actors so them acting in English means they get to keep much of their craft and nuance intact. It would definitely be worse if they had speak unnatural Russian.

Those two characters are actually very plausible, including their reactions and respective knowledge about radiation, just remember the context.

This is USSR in the mid 80's, she's young and he's old. So it's very likely that the effects of radiations on the human body were part of her medical training, whereas he probably attended medical school at a time where the cold war was in its infancy and radiation poisoning concerns weren't a thing, or even well documented enough in med schools.
Also they're in a maternity ward, meaning he's a specialist who had very little incentive to update his medical knowledge beyond his specific field of expertise, so it's not unreasonable to think he never bothered with learning about ARS.

I’ll add that carrier life in general sucks, but especially so for the reactor department because of the additional red tape and bureaucracy that comes with being part of the nuclear Navy. Imagine being in an extremely cramped town of 6,000 people, unable to leave, without any weekends off or vacation time, having to share your bedroom with 20 other people and your personal space being relegated to a wall locker and a small space under your bunk. Now imagine that every day is a full work day, you’re woken up each morning by a bosun’s pipe and a half-dead from sleep deprivation voice crackling through an intercom box, and when you’re off shift you’re expected to be studying for qualifications or assisting with some maintenance or cleaning detail, all the while your rulers 15 decks above make sure everyone is doing what they’re supposed to be doing with frequent shipwide intercom calls and bells.
The environment inside a submarine is far quieter due to the nature of the mission, no bells, no whistles, quiet comms, no pounding footsteps on the decks above or below because everyone is wearing tennis shoes. Add to that the peaceful sleep that comes from the lack of getting tossed around by waves, none of that 500 feet beneath the surface. Granted, you won’t be getting much sleep because there is a lot to do, but still, small comforts.
