It was not just common practise. It was inevitable. Once soldiers get into a besieged city, the generals lose total control over their troops. The lines of communications get broken. Orders can no longer reach the troops. Every man is out for himself. Nobody needs to order a sacking of a city. It happens on its own. You need extremely well disciplined soldiers to avoid it, even under the penalty of death.

This is to the point that many soldiers hope that the city won't surrender so they will get a share of the loot. Some soldiers may have even joined in purposely to get a share of the spoils. Some armies were paid entirely by looting without ever seeing a single coin from his overlord.
