Took awhile to get back up to 1 million of rome during the height of the roman empire after the dark ages, but there were still large cities.
You also don't have that as a starting point if you want it to make sense.
The city and it's plausible size and infrastructure is informed by the geography, demographics and history of the world it takes place in. Those aspects are informed by real world analogues. You don't need to take a city in real life and then move it into fantasyland, that can actually make less sense, a city of Rome might not make sense in westeros for example even though it definitely does exist.

King's Landing scenes were shot in a town called Dubrovnik. There's obviously a lot of CGI in those larger shots to make it seem bigger but the walls, streets, houses etc were actually built in 13th century. You can see those round wall towers in a few scenes, completely unedited.
