I much prefer the idea that he was almost worthy, or didn't believe himself worthy, because he was weighted down by his guilt of knowing he hadn't told Tony the truth about his parents and still angsting about not yet saving Bucky. But in Endgame, Cap has both saved Bucky from his programming and made peace with Tony, so now he can truly fight without any regret, making him worthy.
