In the first two Hellraiser they aren't. Over time the Cenobites became overzealous with what to give the box opener, and the degeneracy blurred the line between pain and pleasure where they think that's what you want. Hellraiser 3 and beyond turned them into demons from hell though.

The idea is you start with small things. Like anything that makes you "feel". In hellraiser hell it's the limits of physical feeling (pain). Like the old small stuff just does not work anymore so they go to the limits of known feeling.
In the movie pinhead mentioned that they are demons to some people and angels to others. It's all a matter of perspective.
