It has a lot to do with the relationship of the people involved. Neither Kenny or Spenny are super talented in any sense and that adds a certain desperation to the show.
This is especially true of Spenny who is essentially completely without skill or talent in any sort of form. As was proven when the show ended. Kenny is having a lot more fun with the show than Spenny is, but the beauty is that Spenny can't quit or leave because his career is essentially over the second the show ends.
That's the real beauty of it: that Spenny, an otherwise very neurotic uptight person is forced to deal with the endless amounts of shit Kenny puts him through because in his heart he knows that if he quits that's the end of his career in television. Kenny knows this too, to an extent, but he's got enough know-how to do other things while Spenny just doesn't.
This kind of desperation breeds conflict and what could have otherwise felt very forced didn't actually end up doing so because they both know deep down that they NEED to be there.

It actually makes me wonder if they were ever really "friends" to begin with. I feel more that they were casual acquaintances that functioned together in smaller doses. Kenny as a person just feels like a really cool fun guy to hang out with once every now and then but not on the permanent basis Spenny has to deal with him.
It would not surprise me if the entire show was Kenny's idea though, and that Spenny just needed the work. Which kinda lines up with your theory.

There are. Though what most of them failed to understand was that it was never intended to be a remake of KVS. Which is what a lot of countries did. They just re-did scene-by-scene stuff from KVS and it felt terribly forced and acted.
What they wanted was for other people to use their formula and come up with new competitions and new and clever concepts, but not a single rendition of the show after the original manged to be anything but a hollow cutout.

It's sort of a hybrid of truth and fiction. This is clearly a real freak out and not simple "acting". There might be scripted elements or plot lines in a particular episode but the bulk of it is off the cuff improv, I think it's the key to the show's formula. that they are real enough to actually be shocking
