(There's some federal involvement with roads due to the interstate system, but of course that isn't mandated in any way, and there's some federal involvement in police procedure.) The difference is that the government does not have a mechanism to determine prices. Prices are determined by factors of production, eg the farmer sells beef to the grocery store for $3 a pound because at that price he can feed his cows, upkeep his farm, make a profit, etc, AND other farmers can't undercut him by doing the same thing and selling for less. So the government employee might offer a salary of $100,000 to the doctor and the doctor might take the job, but if doctors are really worth $80,000 a private company will always form and pay a doctor that much because there is a profit to be made (taking customers from the government hospitals). But in a socialist economy the private hospital would be banned/controlled because the government hospital can't compete.
In the US today prices for medical services are skewed because of a lot of factors like insurance regulations, insurance mandates, medical practice requirements, etc

I never know when I'm going to be hit by a speeding car or develop cancer and need expensive treatment. There are people living literally on the streets who need that treatment too. Even if I paid for private health care, what happens if a close friend or relative without health care falls seriously ill? Either I watch them die or I bankrupt myself to help them.
