The movie was his internal struggle dying on the operating table. He was able to come to terms with his death and let go, which is why one of the surgeons says he put up a hell of a fight. He was finally let go and embraced death. People who have spent time in the ICU have described similar experiences.

People can say whatever they want about the ending but this movie is a fucking classic psychological thriller like Silence of the Lambs.

The screenwriter for Jacob’s Ladder is Jewish, we know that “Jewish neuroticism” is a theme in film (Woody Allen, Larry David, etc).
