Book Ramsay clearly terrorizes women because he is a big, ugly, doughy, peasant boy that no pretty girl would ever glance at. Weirdly he is a really good sneak thief, but it's clear that he's extremely insecure about the nature of his birth and status, as well as sensitive about the idea that he isn't a "real" Bolton. Hence why he is exceptionally brutal and eager to skin everything in sight. Ironically, the Lords of the North mention that Ramsay is far worse than any of the historical Boltons or his father (who comes across as ruthless and amoral but not sadistic).
