Assuming DB only got one film before Z, there would need to be major changes to the source material if they wanted to keep the story anywhere near the original.
I'd merge the story beats of the Pilaf Saga into the Piccolo Saga to tell as condense of a story as possible.
Start the movie with a very quick narrated prologue about what the dragon balls are and how they are used. The opening scene would actually be the first wish of the original series, with Oolong summoning the dragon, however I'd swap around some of the characters. As much as I like Oolong, he kinda has no place in a 2 hour runtime and his screentime could be used for much more important characters. That's why he would be replaced with Roshi in this scene, who wishes for a pair of underwear intercepting Pilafs wish.
It's important to make this the opening scene because it easily conveys how important the dragon balls are, while also introducing a bunch of the major players and the secondary antagonist at once, even though it's weird to place it at the very top of the story.
I was gonna type out a whole rough concept for the film from this point on, but honestly it's more effort than it's worth. Basically have the gang chasing after the Dragon Balls against Pilaf who is working for Piccolo. 2nd arc ends with Piccolo finally having all of the Dragon Balls and wishing for his youth, but instead of killing Chiaotsu, he kills Krillin, giving Krillins death more meaning and the scene more shock value.
Overconfident, piccolo agrees to meet up with and fight Goku at the Tenkaichi Budukai where we meet Tien and Chiaotsu (they won't really do a ton in this movie, but it's necessary to know them now for the Saiyan Saga). Goku successfully kills Piccolo who gives birth to JR and the movie ends. Basically this final part would be a combination of the Piccolo Saga and the final Budukai in the anime
This isn't awful but it needs a ton of work.

Dragonball's distinct art style is so tied to it that I think it would be impossible to make a live action Dragon Ball film franchise (because the art style doesn't translate to reality)
