When someone dies their soul goes off if that person is resurrected then their soul comes back to their body. Ben died and was resurrected so many times his soul got messed up. In Spencer's ASM it was confirmed that Connors' son is the same because he has the same soul as his original son.
Don't forget teleportation technology that takes you apart in one place and puts you together somewhere else. Lots of soulless not-originals out there.

It's obviously not 100% true, but I imagine Hickman has coordinated things at an editorial level so that no other comics currently coming out can disprove House of X isn't happening right now in 616. Only notable difference is the number of dead in the Genosha attack, but that may have just been a fuck up by Hickman. Still, all the story beats point towards what he said being true, what with all the "you may have one more life if you make the right choices" Moira thing and Moira being absent from all of the events in House of X.
So how bad is Hickman going to fuck up Magik? Unlike these non-mystical plebs her soul explicitly does not instantly return to her body until she reclaims it.

Yes and no. It was a recursive loop. It was altered, but in the new reality, at the end of WWII, the allies used the cosmic cube to change things back with the altered timeline's Captain America being the only survivor. So it wasn't just Cap's memories being altered, since Captain Hydra actually experienced that reality, but that reality was erased.
