No one has any idea where it started, apparently there were multiple patient zeroes across the world that popped up simultaneously and some hints that the "virus" has come and gone before. Some support for it being supernatural include the virus still being infectious after being on a bullet, which can reach past the boiling point of water, though that could be a case of writer's ignorance. The spread happened quickly because any contact with any infected body fluid will turn a victim almost immediately, and because the Crossed enjoy rape and mutilation, there was a lot of bodily fluids flying around.

That one in the OP is called Smokey and he is one of the few "smart" crossed in existence; he retains enough of the self-preservation instincts of a non-infected to show things like restraint and long-term planning. The guy he's next to is an uninfected he took on as a worker of sorts and the scene in OP is after he'd been friends with the guy for 20 years as the only non-hedonistic idiots around, with the added bonding from both being bossed around by Smokey's wives, a pair of smart crossed that he hooked up with in the hopes of breeding a population of smart crossed that could establish a sustainable civilization.

I thought for sure the visual language was indicating a slap, but the more I look at it the more I see there's no connection between Doom's hand and her face. Ordinarily if Doom had hit her we'd be seeing a tiny spattering of blood or a more violent turn of her neck, but the only connective tissue between Doom and the lady is the ethereal background mist that seems to be wafting off of Doom's mask and face.

Moses was smart, but he didn't have much of the actual patience without being bossed around by Aoilean iirc. Dude ransacked and crashed a fucking cruise liner wasting all the supplies on board, on top of trying to kill Shakey a bunch of times. Meanwhile, the tracker guy can't speak or even walk so we have no clue how intelligent he really was, he just has a really good sense of smell and hearing.
