Taking responsibility for what happened. Note that it's not until the very end that Simba realizes Scar actually killed Mufasa. He spent the rest of the climax believing he was the ultimate cause of it and still went back anyway to try and stop Scar's rule. The actual mistake Simba made back then was running away from his pain and fear, not trusting Scar or getting Mufasa killed. This is what's portrayed in the movie itself, not just in Simba's mind, as the real problem he needs to overcome.
