It's essentially a 'rovk' born of the powers of causality and set to activate for a specific person at a highly specific period of time typically coinciding with their lowest point. It then summons a bunch of Super Satans who offer to make the individual into one of their number. They simply have to sacrifice all those around them, all those they love and care about, first.

We have no idea how powerful they are. They also don't force things. They'll grant the new prospective member all the power they want, but its up to said member what they do with that power.
Only those destined by causality can use one of those things, once activated you get transported into the cover of a heavy metal album, where these hardcore Hellraisers looking dudes offer you a Faustian deal, where you get what you want in exchange for the thing you love the most.
Then you get superpowers and become a fucked up monster that can transform into an evil looking version of your old self.
That's only for the 'normal' Behelit, mind. The Egg of the King like in OP is somethingnonly sent out when a new member of the God Hand is destined to arise.

Joker wouldn't use it because Joker COULDN'T use it. You have to care about what you're sacrificing so it won't work if you're a psycho who just kills on a whim or a joke.
Thing is, we have no idea how powerful they are. That's it. We can't say anything about how they compare to cape powers because the God Hand have NEVER been put into a situation where they're even mildly inconvenienced. It's not a case of "Oh they're just X level" because that would imply we've ever seen them at a point where they need to boast about how powerful their stuff is. Causality and all things in turn are under their domain no matter how much they struggle.
We can't say they're stronger than Death or not because we've never seen a situation where they've had to even lift a finger in effort at something.

now that I think about it there's actually quite a lot of similarities to that scene and Griffith's own sacrifice, was that intentional?
They wouldn't be messing with Death, only granting Thanos whatever more power he would get as an apostle. Whether this gets death into Thanos doesn't really matter to them.

Same rules. You have to love what you sacrifice. An early story arc had a baron sacrifice his heathen cheating whore of a wife but the God Hand would refuse to accept Guts as a sacrifice when the baron offered. Not just because Guts was already branded but because he was an enemy and the baron had no love for him. They wanted his daughter, which he, surprisingly, refused.

We sit and wait and pray that Miura doesn't dissolve into a skeleton before he can get to the next chapter. Threads like these are only made as a last resort when all else fails.
