The enchantment that keeps it from being lifted also keeps it from sinking or breaking through things that it's placed on. Otherwise it would just sink through the Earth's crust when it was put down.
If the surface it's on is big enough for Mjolnir to consider it "the Ground" then you can lift Mjolnir by lifting that.

Truth be told worthiness is just a way to twist the plot around the hammer any way you want it. by having a vague meaning on worthiness means you can say who or not to your idea of what's worthy.

Well three possibilities.
1) The hammer didn't recognize him as another being since the energy flowing through him was made up of it's own lightning.
2) machines CAN lift the hammer, unless they are being used as an extension of the will of a lifeform, so as a machine with his own independent will Vision got off on a technicality that Odin didn't plan for when making the enchantment

So I did a little research into this. Apparently there's some discrepancies between the comic and movie hammers.
So while the comic Mjolnir was forged IN the heart of a dying start, the movie hammer was forged FROM the heart of a dying star. Doing the math on the weight of collapsing star matter, the movie hammer must weigh somewhere in the ballpark of 3 quadrillion pounds, so for to happen something like must be true.
The comic version however has officially stats, and canonically weighs in at exactly 42.3 pounds.

It depends on when in the timeline, because post-Cell games Gohan has some of the chutzpah to lift it but anything before that is a no since his own fears held back his potential. Krillin also nearly damned the world for refusing to kill #18, but that was doing the wrong thing for almost the right reasons so IDK...
