Ultron was the biggest reason but we see Ross showing incidents outside of the ones in AOU that supposedly take place in between films where Caps Avengers were fucking shit up that had nothing to do with Tony. All those incidents piled up on top of the Ultron one and Lagos was the catalyst. AMATW shows us how ineffective the Accords were in stopping super heroes from doing their thing anyway. If they couldn't stop Scott and Hope who didn't sign I doubt they'd be able to stop a full team who did. All not signing did was split the team apart and make over half of them fugitives.
I don't know but its annoying as someone who reads a decent amount of manga. Capeshit action is garbage most of the time, your reading it for the characters/plot. Its also monthly instead of weekly like many action manga so you need to use the pages for story or you risk pissing off your audience for "padding"
