Its something primal. Its fine in cartoon form or from a distance, but up close clown makeup can just look creepy. Clowns have provoked that reaction since as long as there have been clowns, so whatever it is isn't cultural, its instinctual.

I think it might be a combination of the pale makeup being similar to the pale skin of a corpse, and then their funny walks being an unnatural way of moving that triggers some people. They are basically zombies with the rot not set in yet, so if you are sensitive to corpses you are probably sensitive to clowns?

I've delved into Clowning, and the problem with the make up is that it's designed for the circus.

The Auguste face paint accentuates the eyes and mouth of the performer, so that people in the seats can see the expressions made by the clown during their act, when they have the spot light on them.

Just... when you're in a darkened circus tent with a single point of light focused on a figure some distance away... it's a lot different then if they're only 3 feet away.

But most modern clown make up for closer performances is different then what you see with circus performances.

I had a lucid dream last night, playing a movie that involved a nightmare clown girl.

Some little boy is run over by a snow plow, and left in a snowdrift at the side of the road. Somehow that translates into the boy ending up in a parallel dimension that is populated entirely by nightmare beings that resemble things that children might irrationally fear.

The boy is met immediately by some weirdos that frighten him. A clown girl chastises the beings that scared the boy, and sends them walking. Much older than the boy, the clown girl is roughly an older teenager. Clown girl is mute, and attempts to communicate with an old novelty horn that is clearly worn and sounds rather spent. She has a basic hobo clown outfit, which was once rainbow but is very faded. Tattered shoes, tattered pants and blouse. Suspenders holding up the very oversized pants. Sharp nightmare teeth, which she doesn't realize are creepy when she smiles. Rainbow hair, white skin, blood red eyes. Red large ball-like nose with nostrils because it is her actual nose. Runny black eye makeup. Normal lips but colored black. The clown girl spends some time trying to cheer the boy up. She performs rodeo lasso tricks, some magic tricks with cards and unending handkerchief ropes, and eventually gets into a routine of juggling flaming bowling pins. She can paint shapes with the trail of flames. Circles, eights, hearts. The finale of the act she somehow uses the flames to render the silhouette of Abraham Lincoln.

A bow for applause, everyone finally happy. Then suddenly the boy is kidnapped by some shadowy figure that lives in an ominous castle. The castle is still lodged in the middle of the city, with high-rise buildings surrounding it as if somehow the castle owner had just failed to sell when gentrification hit.

Somehow the clown girl retrieves the boy, by using her various clowning skills and acrobatics. They make a run for an exit portal, and get back to the boy's world...

... Much to the surprise and fright of the clown girl, she is stranded on the boy's side, and neither of them knows how to send her back home. Basically the beginning of a sequel. The boy tugs the clown girl home with him, and drags her before his parents. The parents don't know what to make of the boy's story, but naturally the authorities are contacted, thus toting the clown girl off to be scrutinized by experts.

Law enforcement and social workers find she can understand any language they care to speak in, but she never answers verbally, and only tries to honk the old horn and make gestures. They are surprised that she is actually not wearing makeup, so her skin and nose are simply naturally the clown colors they appear to be. DNA tests are confusing, suggesting that she is somehow a match with a French girl that was buried in the 14th century. Eventually the girl is sent back to the home of the boy, where the parents decide to do their best to help this strange girl acclimate to her surroundings.

They are a bit disconcerted that she could wear such large pants while not earing anything underneath, but decide the solution for everything is a trip to the department story anyhow. They pick out new clothing for clown girl, and of course she gravitates toward bright colors. Hair is given a more contemporary styling. After some confusion in the cosmetics department, they finally decide the only thing they can really do is paint over her unusually coloring with a white base that matches her basic white pallor. Her nose is still someone clown shaped, but the white makeup lets the color blend.

They plan to send her to school, and I kinda woke up around that point in the dream.

That's literally it. It's supposed to be a mystery, and I don't know if it will ever be fully explained.
Basically some people wake up one day with the "twirly arm" affliction without any (known) reason, but they can control it with a special mask device (and make them retract into normal looking arms if they want).
The ARMS fighters are just in a special boxing league for those people and wear wacky gloves to fight.
