From what I've gathered, it's because they had this "literally a fangirl of the franchise's most iconic character" character and what they chose to do with it was "psycho stalker" instead of anything remotely positive. And then her love literally kills her.

They hadn't worked out what Darth even meant until well after the OT trilogy was over. The only time I can think of him being called Darth was in A New Hope, and that was weird because Obi-Wan treated it more as though it was his first name rather than a title (because again, Lucas didn't decide it was a title until later).

Looking in to it a bit more, I don't think they even decided that 'Darth' was a title for dark Jedi until the release of the Phantom Menace. Until then, Darth Vader was basically just considered Anakin's Dark Jedi name and not an actual title.
