It's not really Grand Design because after only one issue everything has diverged. Issue 1 was pretty faithful for the most part, but we're only a few pages into Issue 2 and everything has already pretty wildly diverged as a result of the real time aspect causing things to go differently.
There's differences in Grand Design to be sure, but not to this scale.
Eh, he wouldn't really be any older than he was in MC2 by that point, and MC2 Peter was still fit and spry. The reason he stopped being Spider-man was due to an injury, not due to age. His age wouldn't really be showing and slowing him down until the last two issues when he's dealing with stuff like the Iraq War and Spider-verse.

Tony didn't come out as Iron Man until late '90s/early 2000s.
Y'know, barring that one time where everyone found out and he proceeded to mindwipe the world of that knowledge. Which, in hindsight, I'm not sure why it took Reed, Strange and Tony all working together to do the same for Peter when Tony had done the same thing years earlier.
