I think they always meant "First Avenger" to mean that Cap was the first of them to become a superhero, while Thor was still a spoiled warrior prince, fighting only for the thrill of it, and for himself.

There actually isn't a "First Avenger" in the sense of first on the team, because the team evolves as an idea over the course of the Avengers movie. Fury approaches Tony first, but then tells him in IM2 that he only wants him on as a consultant. Cap is brought in on Fury's request, but isn't really told of Fury's dream of a superhero team, just that they need him to help recover the Tesseract. Romanoff is there in her capacity as a spy and employee, Barton is security on the Tesseract, Banner is brought in under the pretense they just want him for his scientist abilities. Thor just shows up because Loki's involved. After things go to shit, fury tells Cap with Tony there about his super-team idea. Presumably, it's the first time Cap's heard about this. What's important to not about the next part, is that several of the heroes go to New York semi-independently of each other, all to get revenge on Loki or save humanity. It's Tony who first says they really are all a team, that they're all called "The Avengers" and they're coming for him, to stall and threaten Loki. It's kind of a bluff really, he has no assurances they're all going to show up, he's relying on their previous shshows of heroism and/or grudges. You could say that makes Tony the first Avenger, since he's the first to claim he's on a team with that name, but he includes the other Avengers as well, right off the bat. The Avengers happened because they had to happen, because a threat emerged. There is no real first Avenger.

I don't think Captain Marvel was supposed to be an Avenger. I interpreted it more as "we need our own team in case aliens attack and she isn't there to save us". Of that team the first person he thought of was Captain America. He might also be the first person to officially join the team but that depends if he was unfrozen before or after the events of Iron Man. Also we don't know when exactly Black Widow and Hawkeye were chosen for the team.
