Both Carol and Thanos resisted it because the writers who handled it (Aaron and Cates) don't understand how it works, specially Cates. Though the Penance Stare seems to work more on actual sinners, and should work consistently like that. Otherwise none of the Avengers would be able to withstand it, to be quite honest.

In the comics, Ghost Rider didn't do this, Doctor Strange did by casting a spell that made Galactus be attacked by the ghosts of all the aliens he had killed, the typical Deus Ex Machina role he had back in the day. Why Strange has never used this spell since is another issue.

At this point the Penance Stare is just an imaginary barrier that's supposed to be unbreakable but gets resisted if you're cool/bad enough. To be fair its always kind of been nebulous, and when tossed around by people who don't even know Ghost Rider that well it just kinda becomes whatever.
