I think Peter would be super fertile. The issue would be he never slept with MJ until they were married, and he wished away that marriage as well as their unborn child. His off & on again fling with Felicia never lead him to sleep with her, he got to like 2nd base maybe 3rd once.
He should have multiple kids in his 30s. With age comes maturity, with acceptance that his Aunt May is old AF and has lived a good life. That making a deal with evil supernatural entities for a few more years with her isn’t what she would want.
So I think he would go to the only man who has the most dealing with Mephisto, DOCTOR DOOM. However one can’t simply webswing into Latveria. So he goes to the only person DOCTOR DOOM would consider a good friend, Dr Strange. Together with DOCTOR DOOM & Dr Strange, Spidey would nullify the contract. However, it’s not without consequence.

This is a weird question, but do Peter's kids inherit his resistance to radiation? I'd imagine they don't, right? Since he has that power because of his radioactive spider blood and if he passed on his radioactive spider blood to his kids, the pregnancy would likely kill MJ or whoever.

Sable would be a reverse Talia Al Ghul. Impose a Batman like hell training to the kid and field missions, but actually being caring and even overprotective and worrysome when her kid is solo.
More times than not she would pick up Peter to join them to have a "family trip" dethroning some dictator, helping rebels in fucked up countries or simply hanging around to have some bonding time between Spidey and kid.
Sable can be a little overbearing and clingy, since their child haven't powers, so kiddo like to spent some time with Peter in NY to have some freedom, hanging around with daddy, create his/her own tricks and gadgets etc. (cue to Silver Spider forging a strange master/student bond with Mysterio).

To add some twist. Every AU Parker kid have spider powers, so, by some factor, Peter and Sable's kid is plain normal and he/she actually feels somewhat unsuitable in comparison to the parents, overcompensating with training, gadgets and field experience (a little of Sable's overachieving nature mixed up with Peter's own wish to always search approval and prove himself).
Kinda a wish of mine see a story with a depowered Peter still trying to be Spider-man, but using gadgets and wits to replicate his powers.
Im thinking the kid would also be a thief, with spider powers at that. But instead of riding his mom coat tails he goes for a Gentleman Thief approach like P5 Joker (he wouldnt use a spider costume as to not draw suspicion and bc he has no conection with peter at the time).
If Peter also had a child with MJ then maybe spider-Joker could be of the child's rogues
