Not every hero needs a recurring villain that is associated with them more than any other villain, repeat offender or not, Joker was that for original Batman, giving Terry a constant Joker would just be rehashing Batman even more than just the premise of "New teenage Batman in the future who is working for Bruce Wayne after getting some personal tragedy that causes him to be Batman in the first place" which detracts from his character.
Plus I think they wanted to be at least slightly realistic with how radiation works, so he'd end up dead from it sooner or later.

It's great because the entire series up to that point has been focused around "games" and we're told over and over how bad it is to "lose" a game, but we never see it. 2 whole seasons of buildup and conditioning where we're totally used to the good guys winning games, and then we get hit by the gut punch of the hero losing a game at a point where they're already fucked and you're triple sure there's no way they'll make him lose since the situation was already desperate.
