I remember a Minecraft mod where you could have kids and if you played in hardcore and died, you played as your kid and if you didn't have a kid to carry on then it was game over.
I thought that was a good idea, with so many roguelites and survival games around I thought it would be common by now.
You could tie it into gameplay even further if you bred with someone that gave your kid good starting stats or increased skill learning speed based on the genes carried over so you could choose risking not having a kid at all and getting a game over trying to woo someone good or go the easy(hard) route and look for the character no one likes that has terrible genes and have a kid that's probably worse than their parent. The challenge would be having to age the existing NPCs every time you die and generate new ones to replace the old and make it seem like a natural thing and make sure all NPCs don't end up seeming the same personality-wise so you have to spend quite a lot of time writing and programming personalities and reactions to pretty much everything for each personality type and personality "hybrids".

Yes if you ignore shit tons of text with backstory in various notes and letters lying around. Levels are well-designed, look like actual places where people lived and interesting to explore (which is why it's often compared to Arx Fatalis), weapons (with alt fires) and enemy roster are pretty decent, movement and gunplay are literally just modernized Doom (it runs on GZDoom).
