Native Isekai means fantasy world someone isn’t transported to. They were born in the fantasy game world.
Isekai literally means "another world". Isekai genre is about some action happening in another world. Native isekai subgenre is about action in another world with protagonist being the native of that world (opposed to the MC teleported/reincarnated from our world).

Lodoss is an original creation conceived for the express purpose of skirting copyright law so its Replays could be published.
If you published a replay of your d&d campaign the TSR fairies might grace with their presence and whisper lovingly in your ear that Forgotten Realms is copyrighted material. Creating your own setting sidestepped that issue.
